HEAR 2012 Applicant Alert
HEAR HSE Forms for applicants who have or are dependent on parents' and guardians' with Medical / GP Visit Cards in date on 31 December 2011, should be brought to local HSE Offices for completion.
Applicants should NOT send forms to the HSE Primary Care Reimbursement Service in Finglas.
The HSE have informed the HEAR scheme that local HSE offices should continue to complete these forms on behalf of HEAR applicants. Applicants who are experiencing difficulty with the completion of their forms should contact a HEAR representative immediately.
